How Long is Rehab following ACL surgery?
12 months officially, followed by maintenance work indefinitely.
When can I return to sport?
If clinical milestones are reached in rehab and the training setting, approximately 12 months.
How often will I have to do my knee exercises following ACL reconstruction?
Daily for the first few stages (weeks 1-6 & possibly 6-12). Eventually as more load is applied, akin to a genuine strength program, then frequency will be reduced by your treating physiotherapist (ie, to 3-4 days weekly). ACL rehabilitation done right is a big undertaking and investment in your long-term health. This should be well understood prior to surgery.
How Often do I need to attend physiotherapy?
Twice weekly for the first three weeks, then once weekly for a few weeks, then as indicated by your physiotherapist in most cases.
What do I need to be careful of in the early days post-surgery?
Your physiotherapist (and post-operative team) will educate you on:
- Monitoring for possible infection at the surgical sites
- Monitoring for possible blood clots (such as Deep Vein Thrombosis) post-surgery
- The early vulnerability of the new ACL and the surgical graft site (typically hamstrings or quadriceps).
Should I buy or rent anything for post surgery (ie, a cryocuff or an Electric Muscle stim unit)?
It is advisable to get a cryocuff to help with management of swelling for the first several weeks post surgery as this is a very important piece of acute care.
A home e-stim unit is also advisable as it can help us improve quadriceps firing with certain exercises during rehab and can be done daily at home instead of only when attending physiotherapy sessions within the clinic.

Island Health offers a voucher program for local arthroplasty patients, providing 12 free/covered physiotherapy sessions for total knee replacements and 3 free/covered sessions for total hip replacements. Eligibility for this program can be confirmed by contacting Island Health directly or your surgeon’s front office.
Similar to ACL rehabilitation, we emphasize the importance of a prompt start to your recovery journey following arthroplasty. A thorough assessment within the first week post-surgery allows us to evaluate the area, prescribe tailored exercises, and apply necessary treatments and modalities. Despite initial discomfort, restoring movement and strength begins immediately, setting the foundation for a successful rehabilitation process.
